What Does a Semen Analysis Measure?
Semen Volume The total amount of fluid produced in a single ejaculation. Low volume can sometimes indicate a blockage or a problem with the seminal vesicles.
Sperm Count and Concentration How many sperm are present per milliliter of semen, and in total. A higher count generally improves the odds of fertilization, though count alone doesn’t tell the whole story.
Sperm Motility The percentage of sperm that are moving, and how well they move. Sperm need to be able to swim effectively to reach and fertilize an egg. We look at both the proportion of moving sperm and the quality of their movement.
Sperm Morphology The size and shape of the sperm. Even in healthy, fertile men, a significant percentage of sperm have abnormal shapes. What matters is whether the proportion of normal-shaped sperm falls within an acceptable range.
Signs of Inflammation Elevated white blood cell counts in semen can indicate infection or inflammation, which can impair sperm function. This is often treatable once identified.
Beyond the Basics: Sperm DNA Fragmentation Testing
A standard semen analysis tells us a lot, but it doesn’t tell us everything. Sperm can look normal under the microscope and still carry damaged DNA. That’s where sperm DNA fragmentation (SDF) testing comes in.
DNA fragmentation refers to breaks or damage within the genetic material inside sperm cells. Even when count, motility, and morphology appear normal, elevated DNA fragmentation can make it harder to achieve or maintain a pregnancy. It has been associated with unexplained infertility, recurrent pregnancy loss, and lower success rates with certain fertility treatments.
Common contributing factors include oxidative stress, varicocele, smoking, environmental toxin exposure, advanced paternal age, illness with high fever, and certain chronic conditions.
